「have to do with」は「関係がある」という意味です。「what」を使わずに「Does it have to do with ~?」という文なら嫌味なしで「～と関係があるの？」と普通に聞いている感じですが、「what」を付けて「What does ~ have to do with ~?」だと嫌味っぽく「何の関係があるの？」、つまり「関係ないでしょう」という言い方です。
たとえば
Does it have to do with the project?
（プロジェクトと関係しているの？）
What does that have to do with the project?!
（プロジェクトに何の関係があるわけ？）
